A change of name must be recorded to enforce a patent application / patent.
There is not time limit for recording a change of name after the change has taken place.

An official certificate by a public officer or a certified copy thereof showing the change of name must be filed in order to support a request to the Office to record the change. Notarization, legalization of the supporting document is needed.

A simple signed POA must be field. However no new POA must be submitted
if the recordal of change of name is filed by the attorney/agent of record.
